Your doctor says you have a prolapse or slipped disc at L4-L5 and L5-S1 level. You are told it is because of the heavy weight you lifted. Have you ever thought that the construction worker who daily lifts much heavier weights rarely gets a slipped disc? “Then why should you?”


More than 90% of spinal injuries are cumulative type. It means that they do not develop overnight. There would be occasional momentary pain or tiredness or stiffness in the back. Over a period of time the frequency and intensity of this pain increases, if ignored. Also, the body exhausts its capability to compensate. Under such circumstance, even a small movement of lifting a chair or bending down to pick up a pen can cause severe pain.


The spinal disc degenerates due to many abnormal pressures. The postures while sitting, standing and also while doing activities develop repetitive strains in muscles and ligaments of the back. Under such circumstance, people who have weak outer layer of inter-vertebral disc develop disc bulges.


Factors responsible for spinal disc pains

It may be one or many of the following:

  • Abnormal postures of sitting
  • Repeated movements of bending
  • Prolonged sitting hours at work
  • Sudden change in the level of physical activity
  • Sudden gain in body weight
  • Chronic knee pains
  • Osteoporosis
  • Diabetes
  • Thyroid disorder
  • Lifting heavy objects in presence of weak shoulder muscles

All of these factors influence the normal function of muscles which may eventually develop triggers or tightness and weaknesses in it. This causes muscle imbalance between the left and right side of body or upper and lower halves of the body. Because the pain is not that severe, you tend to continue with your daily activities. What you feel is only some stiffness, or some pain which gets relieved after rest. The continuation of activity under such circumstances slowly deteriorates the health of joints, ligaments and discs. It is now, that even with a minimal activity, you may develop slipped disc or acute lumbar strain. And it is because your body has exhausted all the compensatory mechanism, you feel the pain.


Surgery-not a cure

Now, you may also understand why surgery in such cases, do not give lasting relief. The surgery involves removal of the prolapsed part of disc. This may ease pain, but it does not help you with a permanent cure. Though you may feel a relief for some time, but you again feel the pain because the root cause of prolapse disc is not attended.


These days there is a new trend being noticed. Following surgery, the surgeon fails to direct the patient to a physical therapist for a complete rehabilitation. Some exercises are taught by the surgeon himself and the patient is left to perform them according to his/her convenience. Concept of assisting recovery to normal life is missing from the protocol. Such patients continue to have a compromised quality of life though pain may not be felt in regular activities. Fear and misconception are identified as the major causes of chronic back pains. Unfortunately, in the absence of specialized post-surgical physical rehabilitation, patients continue to foster their beliefs.  


Also, there are chances of failed back surgery where there may be no relief or increased pain. Also, surgery has its own risk of complications.


It is only 5% of back pain cases who actually require surgical management.


What we do?
We do not advise any bed rest for prolapse disc. It is an old theory which is proved wrong by various research reports. OUR MANTRA Keep moving as much as you can; this hastens the recovery and prevents the pain from becoming chronic.


We have a thorough evaluation of your body structure, mechanics, your habits, your life-style and your nutrition. We look into the root cause of the slipped disc. We also analyse the cause-effects of our findings and plan the treatment to restore the body functions. This process takes time, but it ensures a lasting results. This does not mean that we do not address pain. Our treatment approach is two-folded:

  1. Acute pain relief: by acupuncture, iontophoresis, kinesiotherapy, very minimal medicines (only if required)
  2. Correct the slipped disc: DRS technique is developed at Total Rehab by Dr Alpana (PT). DRS signify disengage, relocate and stabilize the disc. All the factors responsible for prolapse of the disc are identified and rectified (disengage). Then with the help of exercises and kinesiotherapy, and other physiotherapy modalities the disc is relocated to its original position (relocate). The last step of DRS is stabilization, which is to prevent further such episodes in future.

How many days of treatment are required?

Depending on the factors responsible for prolapse, patient may require between 15-days to 45-days of therapy. It also depends on the duration of prolapse pain and its impact on functional activities.
